OverviewProvides direct care to persons requiring inpatient skilled or acute rehabilitation. Assists Registered Nurses and Therapists to meet the safety and personal needs of patients following a change in their health or functional status. This Patient Care Tech, specializing in rehabilitation, finds personal value in working with those with functional and cognitive disability and chronic illness. This role acts in a manner to bring hope and support to persons to function at their highest independence. The Post-Acute Patient Care Tech contributes to the person's successful discharge to the community, to assisting the person served to gain the highest level of independence possible, in the prevention of health complications, and overall satisfaction of services provided. ResponsibilitiesPerforms patient care & treatments under supervision of a RN. Provides & documents nursing care including, but not limited to, activities of daily living, exercise & ambulating, specimen collection, respiratory support activities, patient & family teaching as directed by the RN. Lifts, moves, positions & transports patients. QualificationsHigh school diploma or equivalent. Minimum of 120hr CNA certification required, or completion of at least one clinical rotation of an accredited nursing program AND be listed as Eligible on the Iowa Direct Care Worker Registry. Indicate on your application which CNA certification you possess (120 hr or 150hr) and/or that you have completed the appropriate nursing program clinical rotation. Candidates must successfully complete a post-offer, pre-employment strength and agility test at Medium Level of Work. Younker Rehabilitation -- Must be 'eligible' on the Direct Care Worker Registry of Iowa within 3 months of hire. Inability to attain 'eligible' status will result in termination of employment.
