RN - Case Manager - Full-time

Objective:

The Registered Nurse is expected to perform a variety of client services duties in the area greater Martinsburg, WV / Winchester, VA area. The Registered Nurse trains and supervises caregivers in the provision of personal care services in accordance with State requirements. The Registered Nurse is conducts IN-HOME service calls, personal care assessments, and supervisory/quality assurance visits in order to provide the highest quality service to clients.

Primary Responsibilities:

• Train in the provision of personal care services in accordance with State requirements
• Ensure continuing compliance with State licensure requirements for personal care
• Conduct IN-HOME service calls and initial RN assessments with prospective/new clients
• Meet and or exceed targeted closing rates
• Develop and maintain positive relationships with all clients through supervisory/quality assurance (QA) visits, phone calls, cards, and special events
• Conduct supervisory/QA visits with clients in person or over the phone according to the QA frequency guidelines
• Administer drug testing program for all new applicants and employees as needed
• Monitor, mediate, and log client activity and follow up on the assignments and client services
• Counsel caregivers on supervisory/quality assurance concerns after visiting clients and provide RN supervision to address concerns as appropriate
• Conduct client introductions as scheduled
• Enter and maintain accurate client records
• Maintain regular attendance at the office to execute job responsibilities
• Demonstrate open and effective communication with owner and colleagues

Secondary Responsibilities:

• Correspond with clients on holidays and birthdays
• Participate in meetings
• Perform any and all other functions and responsibilities deemed necessary

Education/Experience Requirements:

• Registered Nurse (RN) license - have or ability to acquire WV & VA
• Must possess a valid driver's license
• AAS/AS/AA degree - does NOT require Bachelor's Degree
