Continuing Care Clinic in Midland

We are looking for a dedicated and motivated Advanced Practice Provider to help grow our Continuing Care Clinic in Midland!

• Full-time
• Monday-Friday, 8:00 a.m. – 4:30 p.m.
• After-hours phone call required
• Responsibilities include managing medications upon discharge, diagnosis and treatment of illness, physical exams, ordering and interpretation of test results, performance of pre-operative physical exams and care for patients who have an acute illness
• Provide care for patients that are discharged from the hospital or emergency room who are without primary care

Requirements

• Graduate of accredited Physician Assistant or Nurse Practitioner Program
• Experience within primary care preferred, new graduates will be considered
• AHA approved BLS
• Active State of Michigan License or Eligible to apply
