Role Overview

As a Primary Care Physician, you will conduct in-person comprehensive assessments and collaborate with the IDT to devise tailored care plans for each participant. Under the direction of the Medical Director, you will oversee participant care within nursing homes and hospitals, handling admissions, discharges, and coordinating with medical specialists to guarantee optimal care. You will also provide guidance and support to Nurse Practitioners and other IDT members.

Key Responsibilities:

• Conduct assessments, planning, primary care, and ongoing medical coverage for participants.
• Collaborate with community-based physicians and the IDT for effective discharge planning from acute or long-term settings.
• Participate actively in IDT meetings, communicate participant changes, and coordinate 24-hour care delivery.
• Maintain participant care in nursing homes through regular visits based on needs.
• Join a rotating schedule for night and weekend calls.

Qualifications:

• Required: MD or DO with successful completion of an accredited residency program.
• Board Certification: Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics preferred.
• Active California Physician License without restrictions.
• State CDS Certification and DEA Registration.
