Reeves Foundation
At Reeves Foundation, we provide residential medical care and long-term support for individuals with developmental disabilities in community-based settings throughout Arizona.
Our nurses play a vital role in supporting medically complex individuals through consistent, hands-on care and meaningful long-term relationships.
This is not a facility-style nursing position. Our nurses become deeply involved in the daily wellbeing, stability, and quality of life of the individuals we serve.
With established locations in Mesa, Central Phoenix, and Surprise, we are seeking compassionate and dedicated Weekdays LPNs to join our team. Our nurses play a vital role in meeting the unique needs of our members and promoting a high quality of life. They are empowered to build meaningful partnerships with patients and their families, who trust us with their care.
Responsibilities
• Medical oversight of Direct Care Professionals
• Ability to work independently with minimal supervision
• Strong organizational and time management skills
• Effective communication and interpersonal skills
• Dependable, flexible, and committed work ethic
• Delivering compassionate care that promotes members’ dignity and well-being
Essential Duties
• G-Tube and catheter changes
• Medication and MAR management/oversight
• Trach care
• Staff training and development
• Conducting daily head-to-toe assessments
• Ensuring compliance with follow-up care after appointments
• Communicating patient needs to providers and executing physicians’ orders
• Proactively identifying member needs and initiating new orders with physicians
• Maintaining accurate documentation using an electronic medical record system
• Updating patient information and ensuring accurate record-keeping
• Providing member transportation to and from home and within the community
Licensing and Certifications Required
• Current, unrestricted Arizona PN license
• Arizona Fingerprint Clearance Card
• CPR and First Aid Certification
• Article 9 Certification (within the first 30 days of hire)
• Prevention and Support Certification (within the first 30 days of hire)
Position Details
• Job Types: Full-time, Part-time, PRN (Weekdays Only)
• Serves individuals with developmental disabilities, assisting with vocational training, daily living skills, and more
• Adherence to company policies and procedures is essential
• Timely and regular attendance is required to ensure adequate staffing and meet the needs of individuals served
