Sun River Health is currently seeking full-time Psychiatric Nurse Practitioner to join our team supporting the Beacon Health Center site located in the Beacon, NY.nbsp;/p pnbsp;A Psychiatric Nurse Practitioner provides psychiatric clinical services for patients. Performs job duties in compliance with systems and job specific standards.br nbsp;/p pEssential Duties and Responsibilities:/p ul liConsult with MD/Psychiatrist/LCSW on patients via eCW, telephone encounter, and/or with patient visits/li liConsult with MD/Psychiatrist on medication management, via eCW, telephone encounter, and/or visit with patient as needed to address BH concerns/li liProvide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals, in support of ongoing care by MD/Psychiatrist/LCSW, with referral back to ongoing care with clinical team/li liParticipate in case conferencing and high-risk meetings with LCSW/MD/Psychiatrist and care teams/li liApproval of treatment plans for patients in BH care at Sun River/li liParticipate in efforts to support Sun River’s primary care and BH integration efforts/li liEnsure appropriate transitions of care with external providers, PCP, hospitals, etc./li liRespond to Sun River clinical teams for crisis intervention/li liInterviews, diagnoses and evaluates patients referred for psychiatric assessment/li liParticipates as a team member in conferences concerned with diagnosis and treatment planning/li liWrites reports and documentation as required/li liConsults with other mental health and medical provider staff regarding treatment of patients/li liPrescribes psychotropic medication as needed and monitors its effectiveness/li liProvides in-service training to professional and paraprofessional staff/li liReviews case records to monitor quality of care/li liMust demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served/li liMust demonstrate knowledge of the principles of growth and development over the life span/li liMust be able to identify and categorize each patient’s age–specific grouping of needs, such as those for infant, adolescent, or geriatric patients/li liMust be familiar with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessary/li /ul pMinimum Education Requirement: Master's Degreebrbr Minimum Licensure: Nurse Practitioner in Psychiatry and CPR/BLS/p pRequired Skills amp; Knowledge:br - Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers.br - Competence in delivery of health care to patients (including preventative health education)br - Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.br - Bilingual in both English and Spanish (orally and written) is preferred/p pSun River Health is a network of over 40 Federally Qualified Health Centers (FQHCs) providing primary, dental, pediatric, OB-GYN, and behavioral health care to over 245,000 patients annually.
