The community LPN will participate in a team approach to meet the needs of the participants in their homes, or in transitional housing. This may include home visits for dressing changes and medication management and then being assigned to a transitional housing location. These are locations where there are 3-8 participants residing in one place and are in this setting to avoid hospitalization and skilled nursing facility visits, keeping them in the community until they are ready to return to their own residence. The LPN plays a vital role in the participants success./ppbr /bRESPONSIBILITIES/b/pullispanPreferred qualifications: home care is preferred but we can work with the right person to gain skills and experience in home care.
