pDelivers coordinated nursing care to dialysis patients ensuring established standards of care. Supports the physician and interdisciplinary team in facilitating patient care with the underlying objective of enhancing quality of clinical outcomes and patient satisfaction. liBachelor's Degree in Nursing (BSN): Preferred/li
liMaster's Degree in Nursing, Clinical Nurse Specialist or Nurse Practitioner: Preferred/li
liExperience in acute care nursing: Preferred/li
liExperience in cardiovascular nursing. : Preferred/li
liExperience in orthopedic case management: Preferred/li

pEssential Functions:/p
ul
liPerforms rounds with the Critical Care Unit and other Inpatient Care Units, as required. Conducts ongoing review and education on dialysis nursing care to staff including all shifts. Ensures elements of the Acute Dialysis program are in place and functioning appropriately. Acts as clinical resource to staff by providing in-services, validating dialysis-related competencies and advising management of training needs./li
liCoordinates all disciplines involved in the care of patients to ensure appropriate, cost-effective and efficient progression through the system and continuity of care. Leads multidisciplinary team efforts to identify clinical problems and needs. Designs processes to implement and manage patient transitions through the system. Evaluates care to ensure desired patient outcomes. Works under minimal supervision./li
liServes as the point-of-contact for the program to both internal/external customers and healthcare providers. Maintains and ensures continued success of the program by complying with quality and regulatory requirements, as outlined by the appropriate agencies. Functions as an expert clinician, consultant educator and leader in the care of the dialysis patient population. Implements day-to-day care management interventions for identified patients./li
liMentors, develops, inspires, educates and leads staff members in patient care setting. Maintains responsibility for enhancing efficiency and cost effectiveness by identifying opportunities to decrease costs, as applicable to the job. Demonstrates subject matter expertise regularly regarding medication, treatment and patient care standards./li
