Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking part-time Nurse Practitioner to join our team supporting the Middletown, NY site./p
pnbsp;A Nurse Practitioner, under the direction of the of a physician, is responsible for planning, evaluating and delivering the health care of patients on assigned service. Emphases preventative health measures and education of patients. nbsp;She/he will perform other duties as assigned and in compliance with systems and job specific standards.br/nbsp;/p
pEssential Duties and Responsibilities for a Nurse Practitioner:/p
ul
liTakes histories, performs physical examinations, gives immunizations (as necessary) and does patient education and counseling./li
liOrders appropriate lab work, draws blood for lab work if needed, and performs some lab tests such as wet prep microscopic exams./li
liPrescribes treatment for minor illnesses, and consults with physician regarding medication, chronic illness and referrals when appropriate./li
liMaintains legible and appropriate documentation and reviews records periodically. Does follow-up of abnormal pap smears and reportable STD’s./li
liAttends case conferences involving patients./li
liAttends continuing education programs to increase skills and knowledge./li
liAttends provider meetings and participates in quality improvement programs and team assignments./li
liIdentifies and categorizes each patient’s age-specific grouping of needs, such as those infants, adolescent, or geriatric patients./li
liFollows OSHA standards of infection control under CDC guidelines endorsed by department, organization, and overseen by Infection Control Officer./li
liProvides safe and successful performance, including meeting productivity, quality and patient satisfaction standards./li

pMinimum Education Requirement: Master’s Degreenbsp;br/br/Minimum Work Related Experience: 5-8 years; 4 years of business office experience, preferably in a large medical practice with increasing levels of responsibility.br/br/Preferred Work Experience: 8+ Yearsbr/br/Minimum Licensure: Valid license as a Nurse Practitioner in New York State and CPR/BLS/p
pRequired Skills amp; Knowledge:br/- Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers.br/- Competence in delivery of health care to patients (including preventative health education)br/- Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.br/- Bilingual in both English and Spanish (orally and written) is preferred/p
