Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered./pulliProvides and collaborates direct patient care services under the direct supervisor of a licensed caregiver. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and implementation of selected portions of the patient care plan as directed under supervisor of nurse./liliAssists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered./liliAddresses and/or directs questions or concerns from patients, families, caregivers, or health care team members./liliPerforms other duties as assigned./li/ul Facility: HonorHealth Support Services Department: Network Float Pool Work Hours: Days or Nights, varied shifts with weekend and holiday availability Shift: Day Position Type: PRN
