Gathers information for both new and established patients such as medical history, diagnostic tests, comprehensive physical examination, behavioral, development and psychosocial assessments.br /br /2. Assesses and identifies patient/family physical, psychosocial, educational, and cultural needs for assigned patient population.br /br /3. Accurately interprets health assessment data and applies findings in directing patient care.br /br /4. Provides ongoing and follow-up care including appropriate pharmacologic and diagnostic treatments and patient/family education.br /br /5. Documents all health data including plan of care and care according to policy, patient/family outcomes, and reassessments and plan modifications.br /br /6. Accurately completes all billing forms, including diagnostic (ICD-9) and treatment (CPT) codes, based on federal and state laws and AC policy.br /br /7. Communicates with referring physicians, health providers, and hospital staff as appropriate.br /br /8. Actively participates in quality improvement activities and the development strategies to improve patient care delivery and cost effectiveness.br /br /9. Disseminates relevant research findings through practice, publication, education, and/or consultation.br /br /10. Participates in AC, local and national professional nursing organizations as appropriate to maintain and share expertise.br /br /11.
