pstrongThrive By 5 is currently hiring Bilingual Service Coordinators for the Early Intervention Program throughout New York City (Brooklyn, Queens, Bronx, and Manhattan). This position involves providing continuous and interactive services that ensures the successful participation of eligible children aged 0-3 and their families in a home-based and/or community-setting.nbsp;/strong/p p/p pstronguEssential Duties and Responsibilities:/u/strong/p pemThe essential duties and responsibilities are not limited to the following:/em/p p/p ulliAssist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP./liliEnsure the IFSP outcomes and strategies reflect the family’s priorities, concerns, and resources; and that changes are made as the families/child’s needs change./liliCoordinate the provision of all services that the child needs and/or are receiving./liliFacilitate the timely delivery of services./liliContinuously seek appropriate services and situations that benefit the development of the eligible child for the duration of the child’s eligibility./liliPerform related duties as required./li/ul p/p pstronguQualifications, Knowledge and Experience:/u/strong/p ulliBachelor’s Degree in health and/ or human services field./liliBilingual Spanish, Bengali, and/or Russiannbsp;preferred, but all languages welcome!nbsp;/liliAbility to effectively communicate with clients and families via phone and email./liliPrevious experience preferred/liliCase management experience preferred/li/ul pbrVisit us at a href="http://www.thriveby-5.com"www.thriveby-5.com/a/p
