Some typical responsibilities include: /strong/p ul /ul ulliEnsure that the individual service plan is followed; administer or supervise treatments prescribed by physician./liliMake meaningful rounds to all residents, utilizing service plans; make rounds with physicians and record visits on charts./liliAdminister all intramuscular, sub-cutaneous injections; perform all in-house testing procedures; take and record vital signs./liliDocument acute episodes or significant changes in resident status./liliWork with Activity Director, DON , and Nurse Aides in carrying out the activities program./liliHandle the preparation and administration of prescribed medications./liliOrder all new medications and refills; administer all medications and chart./liliChart on his/her shift in accordance with facility policy, state guidelines and residents status./liliSupervise serving of prescribed diets and fluid intake; report persistent unresolved problems to the physician and/or DON or appropriate discipline./liliTimely and accurately prepare incident reports./liliTimely and accurately complete admission assessments and appropriate discharge documentation./liliSupervise residents who self-administer medication while following community and regulatory guidelines/liliProvide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living./liliOther duties as assigned./li/ul pstrongQualifications/Skills/Educational Requirements/strong/p ullistrongRequires a LPN license./strong/liliAbility to lift at least 51 lbs on a regular basis and ability to help residents who require physical assistance./liliFluently read, write, speak, and understand the English language./liliMay require experience with caring for residents with memory impairment if assigned to memory care areas of a community./li/ul
