Occupational Therapist For Home Health Visits - Full-time

pApollo is recruiting for an Occupational Therapist to join our Home Healthcare Team. In this role, you will be responsible for providing quality home health visits to our patients in the comfort of their home. We are recruiting for healthcare professionals that want the chance to really own their work and have a say in the organization where they work./pbrObjectives:ul liEvaluates the patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care./li liFor patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Apollo policy./li liDevelops a treatment program and establishes goals for improved function. Communicates plan of care to referring physicians and other Apollo professionals./li liMay teach new skills or re-train patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Apollo policy./li liMay fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function./li liMay train patient in the use of prosthetic and/or orthotic devices./li liMaintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits these records in accordance with Apollo policy./li liMaintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care./li liAttends and contributes to in services, case conferences, and other meetings as required by Apollo policy to ensure coordinated and comprehensive plans of care for the patients of Apollo./li liIdentifies patient and family/caregiver needs for other home health services and refers as appropriate./li liInstructs the patient's family/caregiver and other Apollo health care personnel in the patient's treatment regimen when indicated and appropriate./li liSupervises certified occupational therapy assistants according to Apollo policy and state regulations./li liWhen therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks./li liParticipates in discharge planning for patient./li /ulpstrongRequirements/strong/pul liGraduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association./li liRegistered with the National Registration Examination of the American Occupational Therapy Association./li liTwo years of clinical experience as an occupational therapist./li liDemonstrates good verbal and written communication, and organizational skills./li liPossesses and maintains current CPR Certification./li liAbility to be able to travel to different patients' homes.Discretion in dealing with sensitive and confidential information/li liPrior experience working in a home health setting is preferred, but not required./li /ulpstrongBenefits/strong/ppFull time employees are eligible for:/pul liMedical, dental, and vision insurance/li liLife Insurance /li liPaid time off/li liFlexible schedule/li liShort- and long-term disability/li /ulpAll employees are invited to become members of Obran Cooperative.
