We are a group of compassionate, tenacious, and resourceful people who are dedicated to serving our clients and community./em/p pChildren�s Intensive Outpatient Services supports children and their families in their community in a least restrictive environment using the Wraparound process (WISe). Wraparound works to honor the family voice and choice, identify strengths, build natural supports, and foster self-sufficiency. Wraparound teams provide comprehensive behavioral health services and supports to Medicaid eligible individuals, up to 21 years of age, with complex behavioral health needs and their families. The goal of the program is for eligible youth to live and thrive in their homes and communities, as well as to avoid or reduce disruptive out-of-home placements. Assessment, treatment, and support services are provided in the youth and family�s natural setting, where needs and challenges present themselves (such as the home, school, and community). Families report excellent outcomes including, youth/family stability, increased support, connection to their community, and hope for the future./p pstrongWhat you will be performing/strong/p pWe are looking for a strongCare Coordinator/strong who can provide direct, supportive, and ongoing coordination of therapeutic services to assist children with behavioral health conditions and their families/natural supports. Includes, but is not limited to, case management, treatment planning, resources identification and assistance, identification and development of natural supports, advocacy, service monitoring, and follow-up, collaboration with natural supports and other providers, and individual and group services. Serves as a treatment team member and provides backup to other team members as needed. This position requires a flexible schedule with evening hours both in the office, in the client home, and in the community. This position also participates in the rotation of an after-hours crisis/emergency response including initial crisis assessment, safety risk, and outreach if needed./p p /p pstrongWhat you will be carrying along/strong/p ul liBA/BS/BSW Degree in a Behavioral Science related field strongrequired/strong./li liExperience in medical, mental health, or social services field preferred./li liDemonstrated knowledge and experience with Evidence-Based Practices, or interest and ability to learn./li liAbility to pass pre-employment criminal background check./li /ul pstrongWhat you will be sharing/strong/p ul liAbility to join with, work with, and support children and their families with complex mental health and behavioral needs from a strengths-based approach./li liAbility to formulate an individual treatment plan and apply appropriate rehabilitation and treatment modalities./li liValid Washington State Driver�s License including vehicle and appropriate insurance./li liIndividuals who do not already hold a license or associate license through the Washington State Department of Health must submit an Agency Affiliated Counselor application upon hire and be approved by the DOH./li /ul pstrongWhat we will be offering/strong/p pAs the region�s largest non-profit community behavioral healthcare organization, we have been serving our clients and our communities for over 110 years.
