Locums, Inc. is seeking a Physician for a PACE Program clinic in San Bernardino, CA to provide services elderly patients. Clinic is specialized to supporting frail geriatric patients and keeping them in the community through inter-disciplinary team care. Position is for a minimum 3-month placement with the opportunity to be permanent. Clinic has close access to the San Bernardino National Forest with excellent hiking, biking and outdoor recreation options. About the Opportunity:- Position open immediately- 8-hour shifts, 5 days a week. No evenings, weekends, or holidays required- Clinic is currently closed to patients. Provider will provide telehealth and in-home visits during working hours (75% Telehealth, 25% In-Home) until clinic is fully re-opened- Estimated 5-7 patients/day- Patients are 100% Geriatric - Primary Care Clinic includes providing diagnosis, treatment and medication prescription, immunizations, and follow-up services to local Geriatric population under the PACE program. - EMR is Greenway PrimeSuite - Malpractice Insurance provided- Travel, lodging, and mileage provided Requirements:- Active and unrestricted California Physician License - Clean Background- Active and unrestricted DEA License- Board Certified
