pstrongDesert Regional Medical Center/strongnbsp;is a 385-bed acute-care hospital classified as a stroke receiving center and level 2 trauma facility with an innovative, patient centered and evidence-based Rehabilitation Services Department. Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation, joint replacement amp; spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.nbsp;/p
pstrongGENERAL DUTIES:/strong/p
pThe individual in this position performs psychosocial assessments and develops a plan to meet the needs of patients with complex social issue/problems, and arranges post-discharge services for patients as needed./p
pThis individual’s responsibility will include, but not limited to the following activities: a) assist with adoptions, abuse and neglect cases, including making referrals to state and federal agencies b) participate in the weekly Complex Case Review meeting and the daily patient care conferencesnbsp; c) provide resources and make referrals to community agencies for ongoing counseling and or continued care d) maintain an updated list ofnbsp; providers for post-discharge carenbsp; e) collaborative communication with post-discharge providersnbsp; f) update Case Managers on variance to post-discharge plansnbsp; g) monitor and provide reports to the DCM and hospital leadership as requestednbsp; h) maintain knowledge of regulatory requirements and community resources i) and other duties as assigned.nbsp;Under general direction of the Social Work Manager/LCSW, delivers age-appropriate social work care to patients and their caregivers, in accordance with hospital policies and procedures, and state and federal regulations. The Social Worker II serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources, and other social work services to meet the complex needs of patients and families in the hospital and clinic settings. MSW will seek out supervision as needed with a Licensed Clinical Social Worker. Will use Patient Medical Record and HPF as primary information systems.nbsp;/p
