pPacific Hospice and Palliative Care is seeking a compassionate and experienced Master or Clinical Social Worker who has 2 years of in-home hospice experience to join our team on a PRN basis. As a Hospice Social Worker, you will play a vital role in providing psychosocial support and care coordination to patients and their families, in their San Antonio area homes, during their end-of-life journey. You will collaborate with a multidisciplinary team to address the emotional, social, and practical needs of patients and ensure their overall well-being. Your expertise in counseling, resource management, and advocacy will be essential in enhancing the quality of life for hospice patients and their loved ones. This is NOT a remote position and you must live in the San Antonio area and be able to visit patients in their homes. /p

liPsychosocial Assessment: Conduct comprehensive assessments of patients and their families to identify their emotional, social, and practical needs related to the end-of-life journey. Evaluate the impact of illness, loss, and grief on individuals and families./li
liCounseling and Emotional Support: Provide counseling and emotional support to patients and their families to help them navigate the challenges of end-of-life care. Offer individual and family counseling, grief counseling, and facilitate support groups as needed./li
liCare Planning and Coordination: Collaborate with the interdisciplinary team to develop individualized care plans that address the psychosocial needs of patients. Coordinate services and resources, both within the hospice team and with external community agencies, to ensure comprehensive support for patients and their families./li
liEducation and Advocacy: Educate patients and families about available resources, advance care planning, and end-of-life issues. Advocate for the rights and preferences of patients, helping them navigate complex healthcare systems and make informed decisions about their care./li
liCrisis Intervention: Provide crisis intervention and support during times of emotional distress, bereavement, or other emergencies. Respond promptly to crisis situations and facilitate appropriate interventions to ensure the safety and well-being of patients and families./li
liDocumentation and Record Keeping: Maintain accurate and up-to-date documentation of assessments, interventions, care plans, and progress notes in accordance with organizational policies and regulatory requirements./li
liCollaboration and Teamwork: Foster effective communication and collaboration with the interdisciplinary team, including physicians, nurses, chaplains, and other professionals, to ensure coordinated and holistic care for patients and families./li
liContinuous Professional Development: Stay current with knowledge and best practices in medical social work and end-of-life care. Participate in professional development activities, attend relevant workshops or conferences, and engage in ongoing learning to enhance your skills and expertise./li

pQualifications:/p
ul
liMaster's degree in Social Work (LMSW or LCSW) from an accredited program./li
liCurrent State of Texas licensure or certification as a Social Worker./li
liMinimum of two years of LMSW/LCSW experience in Hospice social work in an in-home setting. br/li
liExperience with Axxess EHR system is preferred, but not mandatory. Prompt record keeping and documentation is required. /li
liStrong knowledge of psychosocial assessment, counseling techniques, and crisis intervention./li
liFamiliarity with community resources, support services, and healthcare systems./li
liExcellent communication and interpersonal skills, with the ability to empathize and build rapport with patients and families./li
liAbility to work effectively within an interdisciplinary team and collaborate with diverse professionals./li
liDemonstrated commitment to continuous professional development and learning./li
liReliable transportation is needed for travel to patient homes and our office./li
