pspan style="font-size: 14px;"POSITION SUMMARY/span/p pspan style="font-size: 14px;"Under the general supervision of the Director of Pharmacy, the Pharmacist will be responsible for having a working knowledge of all areas of pharmaceutical services provided by Coalinga Regional Medical Center, including drug distribution systems, central operations, and IV admixture. They will interpret physician’s orders for medications, compound and dispense prescribed drugs. The Pharmacist will maintain records on all medication orders filled for inpatient use. The Pharmacist will evaluate pharmacological, chemical, or physical incompatibilities in physician’s orders, and communicate the evaluation to physicians. They will participate in distribution of medications to patients as well as educate them on their medications. She/he will dispense floor stock controlled substances and maintain inventory records. The Pharmacist will respond to emergency needs of the department at any time when called upon by the Director or designee. They will advise the Director of malfunctioning equipment, maintenance problems, and any unusual situations, occurrences, conditions, reactions, or effects including those related to drugs, drug requests, usage or security. They must be able to work cooperatively with all hospital employees and patients and encourage and maintain good personal and business relationships with them. In addition to the daily on-site activity, the Pharmacist will be available as needed for on-call service. May assist in the teaching of students and house staff where applicable. The Pharmacist must perform other duties as needed. The Pharmacist may also review the drug regimen in the SNF at least monthly of each patient and prepare appropriate reports. This position does not provide direct patient care. Must perform other duties as assigned./span/p pbr/p pspan style="font-size: 14px;"**ONLY PER DIEM**/span/p pbr/p pspan style="font-size: 14px;"POSITION QUALIFICATIONS/span/p pspan style="font-size: 14px;"This position requires graduation from an accredited School of Pharmacy and CA State licensure. Must meet requirement of 30 hours of continuing education every two (2) year. Must exercise sound professional judgment using initiative consistent with a high level of resident care. Requires the ability to work efficiently and under broad guidelines, and must frequently exercise his/her own judgment in decision making. Regularly works with confidential information. Requires endurance to perform tasks over long periods of work hours. The position requires the ability to perform essential functions without posing a “direct threat” in the workplace./span/p pbr/p pspan style="font-size: 14px;"DUTIES AND RESPONSIBILITES/span/p pspan style="font-size: 14px;"The Pharmacist is responsible for the daily organization and supervision of medication management activities in their areas, including but not limited to provision of clinical and drug distribution services. The employee performs job duties with sensitivity to the patients’ diverse cultural backgrounds/spanbr/p ul lispan style="font-size: 14px;"Preparation and dispensing of drugs in the inpatient setting, including sterile compounding/span/li lispan style="font-size: 14px;"Entering medication orders into the computer system and retrieval of patient data/span/li lispan style="font-size: 14px;"Maintenance and review of patient medication profiles/span/li lispan style="font-size: 14px;"Reviewing and verifying physician medication orders and patient medical records/span/li lispan style="font-size: 14px;"Interacting with healthcare professionals and providers to help promote safe medication use/span/li lispan style="font-size: 14px;"Prevent inappropriate or misuse of medications;/span/li lispan style="font-size: 14px;"Provide clinical pharmacy services (pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN per pharmacy, clinical protocols per pharmacy and pharmacist interventions./span/li lispan style="font-size: 14px;"Supervision of pharmacy technicians, interns, and students (if any)/span/li lispan style="font-size: 14px;"Provide good customer service through compassion and efficiency/span/li lispan style="font-size: 14px;"Inventory and supply management/span/li lispan style="font-size: 14px;"Support for the Director of Pharmacy/span/li lispan style="font-size: 14px;"Ensure accurate record keeping and documentation/span/li lispan style="font-size: 14px;"Basic knowledge of computer systems, Policies and procedures/span/li lispan style="font-size: 14px;"Monthly unit inspections and medication pass rounds/span/li lispan style="font-size: 14px;"All other duties as assigned/span/li /ul
