County Connections is seeking a compassionate and driven Support Coordinator to join our team in the Troy, Mo area. In this vital role, you'll serve as the primary point of contact for individuals with intellectual and developmental disabilities and their families.
As a Support Coordinator, you will manage a diverse caseload and play a key role in coordinating services tailored to each individual’s unique needs and goals. You’ll collaborate with local agencies and providers, advocate for necessary supports, and guide families through the process of accessing services and resources. Working as part of a multidisciplinary team, you'll help determine the right level of support and ensure that services align with person-centered outcomes.
Key Responsibilities:
- Coordinate and monitor individualized support plans
- Work directly with individuals and families to identify needs and connect to resources
- Collaborate with community partners and service providers
- Conduct assessments and participate in team planning meetings
- Advocate for individuals to ensure access to quality services
- Review all monthly reports relating to the individual
- Maintains accurate documentation within the required timelines

Qualifications:
- Bachelor’s degree from an accredited college
- Current, valid driver’s license with willingness to obtain a Class E license upon hire
- Possess a reliable vehicle
- Ability to pass all required background screenings
- Effective written and verbal communication skills
- Proficient in Google Docs and database systems
