Based on state regulations, may perform administrative and/or field/clinical responsibilities while receiving oversight and clinical direction from a Registered Nurse./p h2External Job Description/h2 ul liProvides skilled nursing care/treatment to client’s per Physicians orders and Plan of care /li liAccurately documents nursing actions of all care given, individual’s health status, and communicate with family and physicians/li liDocuments the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs/li liProvides skilled nursing treatments per physician’s orders and within the scope of practice/li liMedication administration and management as needed/li liProvides health teaching of client and assisting client in learning appropriate self-care techniques and safety measures /li liConducts nursing assessment visit as needed/li liReports all incidents per operation procedure and regulatory standards/li liFollows policies and procedures according to operation, state and regulatory standards/li liUses safe body mechanics to prevent personal injury to self or client/li liMaintains confidentiality of patient related information and honor patient rights/li liConsults, seeks advice, and follows direction of the RN in fulfillment of care of the client/li liCompletes Start of Care assessments in person, where applicable and in accordance with state regulations (as applicable)/li liCompletes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation. Completes daily, monthly, quarterly and annual nursing documents as/if required./li liWorks closely with Director or Nursing/Nurse Supervisor and/or Branch Manager in planning, implementing, and evaluating programs and services to address the health needs of persons served/li liAttends and participates in staff meeting, in services, and other meetings as requested/li liEnsures compliance with state/local/payer requirements and that documentation reflects the care provided and meets company, regulatory, and payer standards/li liEnsures the ordering, reviewing, monitoring, discontinuation and, if necessary, the disposal of medications as prescribed by the physician/li liTranscribes or ensures transcription of physician phone orders and other medically related reports/li liEnsures that furnishings, equipment, and supplies are maintained within guidelines established to prevent the spread of disease/li liEnsures that appointments for health related issues are scheduled, completed and that appropriate response to findings are implemented in a timely manner/li liCommunicates with Branch Manager, Customer Service Manager and/or Scheduler when clients are hospitalized/li liCommunicates effectively and routinely with supervisor, ensuring that pertinent information reaches supervisor in timely manner/li liProvides new employee training, annual re-certification training, and competency validation for health and safety related topics/li liMay be asked to cover hours, weekends, and holiday on call to meet the needs of clients/li liOther duties as assigned/li /ul h2Qualifications/h2 ul liDegree in Nursing from an accredited school/li liExperience in administering medications/li liOne or more years of nursing experience preferred/li liHome health care experience preferred/li liCurrent and unrestricted license in the state of the operation/li liCPR professional certification/li liBasic clinical skills/li liProfessional appearance and demeanor/li liGeneral knowledge of computers and the ability to learn industry specific software and documentation applications/li liExcellent oral, written, and interpersonal communication skills with all levels of personnel, internal and external to the company/li liExcellent organizational and time management skills with a keen attention to details/li liCapable of working responsibly with highly confidential information /li liAbility to work effectively independently or in a team environment/li liAbility to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issues/li liAbility to learn and complete various educational and position specific goals /li /ul h2About our Line of Business/h2 pAll Ways Caring HomeCare delivers quality, compassionate, and individualized care and support that helps people in need of assistance stay at home – all while maximizing their dignity, privacy, and independence.
