Provider Requirements:

• License: Current Tennessee State License & Unrestricted DEA
• Board Certified or Board Eligible
• Certifications: Advanced Cardiovascular Life Support (ACLS), Advanced Trauma Life Support (ATLS) & Pediatric Advanced Life Support (PALS) - if ABEM, Life support certificates not required
• Capabilities: Must be able to treat patients of all ages
