Sentara Health is seeking to hire a qualified individual to join our team as a Bereavement Coordinator.
Position Status: Full Time
Position Location: Sentara Hospice Services - Harrisonburg, VA
Overview
Responsible for planning, implementing and maintaining a bereavement program to meet the needs of the families/ caregivers of patients in the Hospice program. Provides bereavement support through grief support, therapeutic and spiritual counseling and through family assessments and consultations with the interdisciplinary team.
Provides individualized counseling and support to assigned patients family members through the grieving process. Meets the regulatory standards and requirements of a hospice bereavement program. Collaborate with volunteer coordinator to recruit suitable candidates for the bereavement support volunteer team. Will serve on Interdisciplinary Group and make home visits as indicated
Education
Bachelor's degree (required)
Certification/Licensure
No specific certification or licensure requirements
Experience
3 years of hospice experience (required)
