Licensed Clinical Social Worker

We have a couple openings for a Licensed Clinical Social Worker to join this innovative team and leader in the health and wellness industry. This position is with an established organization who puts a passionate emphasis on people, choice and well-being.

In this position as a LCSW, you will employ a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. You will identify and resolve barriers that hinder effective care and ensure patients are progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.

You may create member care plans and will need an understanding of the department, segment and organizational strategies, operating objectives to include their linkage to related areas. You will make decisions in regards to work methods, occasionally in ambiguous situations, and require minimal direction and receive guidance where needed.

Patient Types: Adult, Geriatric

REQUIREMENTS

• Master’s in behavioral health field including; Social Work or Adult/Geriatric Counseling
• LCSW is required by the state of TX 
• 3 years working in a clinical, outpatient setting
• **MUST be bi-lingual in English and Spanish with the ability to read, write and communicate in both languages fully**
• Valid TX Driver's License and proof of personal vehicle liability insurance
• Trained in behavioral medicine and health psychology concepts (preferred)
• Trained in social, systems and familial aspects of psychological and medical conditions (preferred)
• Trained in substance abuse identification and treatment (preferred)
• Completed Medicare ID Number (strongly preferred)
