Position Overview:We are seeking a committed Occupational Therapist (OT) to join our team at a Skilled Nursing Facility (SNF) in Milford, Delaware. This role focuses on helping residents improve their independence, functionality, and quality of life through effective and personalized occupational therapy interventions.

Responsibilities:

• Conduct assessments and develop individualized treatment plans based on resident goals and capabilities.
• Implement therapeutic activities focused on enhancing daily living and functional skills.
• Document patient progress and collaborate with the healthcare team to adjust treatment as needed.
• Educate residents and their families on exercises and practices to continue at home.
• Ensure a safe and supportive environment during all therapy sessions.
• Adhere to facility protocols and industry best practices.

Qualifications:

• Master's or Doctorate degree in Occupational Therapy from an accredited program.
• Active Occupational Therapist license in Delaware or eligibility to obtain it.
• Experience in a skilled nursing facility (SNF) setting is preferred.
• Compassionate, patient-centered approach with excellent interpersonal skills.
• Ability to work effectively in a collaborative, team-focused environment.
