Summary:
The Mobile Crisis Management Licensed Clinician serves as a key member of the MCM team, providing in-person crisis assessment, intervention, and stabilization for individuals experiencing mental health or substance use crises in community-based settings. This role involves clinical decision-making, supervision of non-licensed staff, and coordination with community resources to prevent hospitalization or institutionalization. The clinician ensures compliance with NC Medicaid Clinical Coverage Policy 8A and delivers services in alignment with the beneficiary’s Person-Centered Plan (PCP).
Essential Duties and Responsibilities:
- Conduct in-person crisis assessments using NC-SNAP or ASAM Criteria to determine eligibility and medical necessity for MCM services.
- Provide therapeutic interventions, including de-escalation, counseling, and crisis planning, to stabilize beneficiaries in community settings.
- Develop and update Person-Centered Plans (PCPs) in collaboration with beneficiaries, families, and other stakeholders, ensuring goals address crisis needs (Subsection 5.7.1, page 7 of NC Medicaid Clinical Coverage Policy 8A).
- Supervise Qualified Professionals (QPs), Associate Professionals (APs), and Paraprofessionals, providing clinical oversight and ensuring interventions align with the PCP (Subsection 5.6, page 6 of NC Medicaid Clinical Coverage Policy 8A).
- Coordinate with Local Management Entity/Managed Care Organizations (LME/MCOs), emergency departments, law enforcement, and other providers to link beneficiaries to follow-up services.
- Document all interventions in service notes, including date, purpose, intervention details, duration, effectiveness, and beneficiary progress, per Subsection 5.8.2 (page 8 of NC Medicaid Clinical Coverage Policy 8A).
- Participate in telehealth consultations (per Policy 1-H) for follow-up assessments or psychiatric evaluations, using the GT modifier for billing (Attachment A, page 30 of NC Medicaid Clinical Coverage Policy 8A).
- Ensure 24/7 availability as part of the MCM first-responder team, responding promptly to crisis calls (page 35 of NC Medicaid Clinical Coverage Policy 8A).
- Maintain compliance with HIPAA, client rights rules, and NC Medicaid policies (Subsection 7.1, page 11 of NC Medicaid Clinical Coverage Policy 8A).
- Participates in all required training and education as mandated by the specific service line and clinical coverage policy.
- Available for on-call shifts.
- Ability to travel to various community settings, including homes, schools, or emergency rooms, and respond to crises in potentially high-risk environments.
- Performs all other duties as reasonably required and assigned.
Supervisory Responsibilities:
This position can oversee staff.
Qualifications:

Education/Experience
- Master’s degree in psychology, social work, counseling, or a related field, or a medical degree (MD/DO).
- Minimum of one year’s experience in providing crisis management services in the following settings: assertive outreach, assertive community treatment, emergency department or other service providing 24-hours-a-day, 7-days-a-week, response in emergent or urgent situations; and 20 hours of training in appropriate crisis intervention strategies within the first 90 days of employment.
- Must hold one of the following licenses per North Carolina General Statutes (Subsection 6.3.1.1, page 10 of NC Medicaid Clinical Coverage Policy 8A):
- Licensed Clinical Mental Health Counselor (LCMHC) or Associate (LCMHCA)
- Licensed Clinical Addiction Specialist (LCAS) or Associate (LCAS-A)
- Certified Clinical Supervisor (CCS)
- Licensed Marriage and Family Therapist (LMFT) or Associate (LMFTA)
- Licensed Clinical Social Worker (LCSW) or Associate (LCSWA)
- Licensed Psychologist or Licensed Psychological Associate
- Nurse Practitioner (NP), Physician Assistant (PA), or Certified Clinical Nurse Specialist (psychiatric focus).
- Registered Nurse (RN)
- Minimum of 2 years of experience in mental health or substance use crisis intervention.
- Bilingual (English/Spanish) to serve diverse populations. preferred.
- Training in trauma-informed care or co-occurring disorder interventions, preferred.
Required Skills/Abilities
- Expertise in crisis de-escalation, diagnostic assessment, and treatment planning; familiarity with NC-SNAP and ASAM Criteria; ability to work in high-pressure community settings; proficiency in telehealth platforms.

Certificates, Licenses, Registrations
- Valid driver’s license, ability to travel to community locations, and availability for on-call shifts.
- Current license or certification must be maintained if applicable.
