CLTS Care Coordinators serves as the main point of contact in helping families obtain CLTS services necessary to help keep their children safely in a home/community setting./p pThe position is full time, Monday through Friday, first shift, and may require flexibility to meet the needs of the families. Bilingual fluency in Spanish preferred but not required./p pUnder direction, performs work involving the assessment, service planning, and implementation of the service plan for assigned individuals. Performs other duties as required./p p /p pstrongDIRECT SERVICE REQUIREMENTS (Required of all employees providing direct services):/strong/p ul liPromote client independence and growth, consistent with the service plan./li liProvide services using a trauma-informed and person and/or family-centered model./li liProvide services utilizing agency approved and supported evidence-based practices that have been indicated by the funder and/or agency./li liAccurately complete time reporting./li /ul p /p pstrongEssential Duties and responsibilities:/strong/p pThis list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time./p ul liAssessment of functional abilities/li ul liAssess client’s needs utilizing the required assessment tool(s) (e.g. functional screen)./li liRe-assess, as needed, and at least annually./li liInformation to complete the assessment is gained via interviews, collateral contacts and prior records./li /ul liCare/Service planning/li ul liWork alongside the client, their family, his/her supports, and other service providers, develop and implement a comprehensive and individualized service plan./li liWhen required by the program/service, coordinate and authorize services according to the service plan./li liWork collaboratively with clients, service teams, and clinical supervisor to determine interventions./li liCommunicate effectively with team members and providers. When authorizing services, articulate the expectations of the service./li liMake recommendations within the client team and scope of practice./li /ul liCare/Service facilitation/li ul liProvide services to assigned clients in alignment with the service plan./li liCoordinate and facilitate person and/or family-centered team meetings./li liTestify at legal proceedings as required by law and/or role./li liComplete documentation within required timelines and expectations per LSS, regulation, funders, and practice standards./li liMaintain client records./li /ul liApply working knowledge of:/li ul liCasework principles and methods./li liHuman behavior, dynamics of groups, interpersonal relations, and social interactions./li liSocial problems and their effects on individuals./li liCommunity Resources/li /ul liAttend and participate in:/li ul liStaff development programs, including in-service training, staff meetings and professional seminars/li liClinical supervision, when supervision is a program requirement/li /ul liUnderstand productivity expectations.
