Job Summary: LCSW - Behavioral Health Consultant

Location: Elgin, OR

Position Overview: We are seeking a Licensed Clinical Social Worker (LCSW) to join our collaborative care team as a Behavioral Health Consultant. This role involves working closely with the patient s primary care provider and other community mental health, addiction treatment, and alternative health care providers. The Behavioral Health Consultant will assist in facilitating behavioral changes that improve patients' physical, emotional, and mental health.

Key Responsibilities:

• Collaborate with primary care providers to support patients in achieving their health goals.
• Provide assessments and interventions to address behavioral health issues.
• Develop and implement treatment plans in coordination with the care team.
• Educate patients and families about mental health and substance abuse.

Qualifications:

1. Current Oregon Qualified Mental Health Professional with no restrictions; LCSW preferred.
2. Previous experience in physician practices preferred.
3. Knowledge of state and federal regulations regarding mental health and substance abuse.
4. Minimum of 5 years of clinical experience in mental health and/or substance abuse treatment preferred.
5. Current American Heart Association Basic Life Support certification.
