Program: CREST (Community Reporting Engagement Support and Treatment)

Location: New Haven

Salary: $20.00 per hour

Schedule: Full time, 40 hours per week, 1st shift ( 8 AM to 4 PM), Monday to Friday 

Program Summary:

CREST (Community Reporting Engagement Support and Treatment) is a structured day reporting program that provides counseling services, in-depth groups, and intensive case management for people with mental illness referred from the criminal justice system. The program works collaboratively with other programs at The Connection, as well as the Connecticut Mental Health Center in New Haven.  

Position Summary:

The Case Manager 3 is responsible for quality assurance, direct supervision of case managers in assigned area.  Coordinate with the program director, The Connection and contracted employees to ensure uniform standards and quality of service.  Provide supervision with regard to processing and assigning cases.  As well as comprehensive case management services to clients at the Roger Sherman House, including intake screenings, risk/need assessments, development and implementation of individual supervision plans, and liaison with treatment providers and community resources.  Work collaboratively with medical treatment, employment, educational, and volunteer linkages in the community on behalf of clients.

Working within Adult Services, the Case Manager 3 works with Community Justice program staff to help to rehabilitate, educate, and successfully guide clients from incarceration back into the community.

Requirements: 

• Bachelor’s Degree
• 2 years of related experience in the direct service and case management field
• Valid driver’s license
• Regular and predictable attendance is required

Orientation:

If selected for employment with The Connection, newly hired employees are required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
