(TTC) that may be appropriate for the patientrsquo;s family members./liliAttends in-services trainings and educational workshops./liliAttends all required staff meetings./liliCooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc./liliAs appropriate works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed./liliAssist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc./liliProvides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals./liliFacilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc./liliMakes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues/liliProvides regular, comprehensive progress note charting in patientrsquo;s Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other case management activities./liliPerforms other duties as assigned by Director and/or Administrator./li/olpstrongEXPERIENCE/strong/pp1. nbsp; nbsp; nbsp; nbsp; For AOD Programs:/pulliEnrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTPnbsp;/lili1 year experience in a Behavioral Healthcare setting./li/ulp2. nbsp; nbsp; nbsp; nbsp; For all other Programs:/pulli1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff/li/ulp3. nbsp; nbsp; nbsp; nbsp; Bilingual English/Spanish preferred/pp4. nbsp; nbsp; nbsp; nbsp; Valid California driverrsquo;s license and insurance/ppstrongLICENSES/CERTIFICATES/strong/ppFor AOD programs, registration or certification with a DHCS approved state level counselor certification agencies./ppstrongSETTING/strong/ppTarzana Treatment Centers, Inc. is a non-profit corporation, which provides behavioral healthcare services, including chemical dependency./ppstrongEQUAL OPPORTUNITY EMPLOYER/strong/ppTarzana Treatment Centers, Inc. does not discriminate as to race, nationality, religion, gender, sexual orientation and disability in its hiring practices./ppstrongADA REQUIREMENT/strong/ppTarzana Treatment Centers, Inc. supports the Americans with Disabilities Act.
