This position offers a flexible work schedule as well as mixed work locations throughout all Northern Virginia counties including Arlington, Fairfax, Prince William, and Loudoun.  Our Field Nurse in-person assessments, reassessments and quality visits are performed either in clients homes or in various health care settings such as Hospitals, Rehab Centers, or Senior Living Communities.

Local travel throughout all Northern Virginia counties of Arlington, Fairfax, Prince William, and Loudoun is required, mileage is reimbursed based on the prevailing IRS mileage rate.   

Requirements:

• Valid Registered Nurse License in the state of Virginia
• Valid Driver's license and clear driving record
• Reliable transportation

Job Description

Tasks for Assessments, Reassessments and Quality visits include:

• Receive client information from the office staff. 
• Schedule the assessment with the client or appropriate family member. 
• Conduct the initial in-home assessment of new Right at Home clients, with clients and/or family members. Document the assessment and write care plan. Assessment and care plan
• Answer questions from the client about our services. Be able to describe our services in an appropriate and effective way. 
• Fill out service agreements with the client if they have not been completed via DocuSign. 
• Contact the RAH office immediately following the assessment to discuss the results of the assessment. 
• Upload assessment and care plan into the electronic health record within 24 hours. 
• Email a copy of the care plan to client or designated family member. 
• Return copies of signed forms and documentation to the RAH office within 5 business days. 
• Conduct re-assessments visits (including unannounced visits) based on the schedule provided by the office staff. Updates to include, but not limited to: new diagnoses or problems, recent hospitalizations/illnesses, recent falls, changes in gait/balance, cognitive changes, skin condition, appetite, sleep, medication changes. 

Pay Structure:
Field Nurse is compensated PRN for assessments, reassessments and quality visit as well as hourly for office time.
