Job description

• Care Coordination Management for Chronic Care Patients
• Collaborate with payers and medical group leaders to design a strategic approach to lowering healthcare spending.
• Monitor all quality, cost, and efficiency for pertinant initiatives and partnerships on a regular basis.
• Utilize the Chronic Care Coordination Tool to impove quality outcomes and reduce unnecessary tests and expense.
• Perform Medical Chart reviews
• Proactively outreach to members and their families (must have strong phone skills and empathetic character)
• Updating patient medical records and clinical documentation
• Updating immunizations in the Tennessee Registry
• Scheduling consultations and appointments with Specialists and local hospitals
• Triage nurse calls
• Follow up on labs, orders, referrals and prior authorizations
• Other duties as required

At least 1-2 years of medical office or pediatric experience preferred

Licensed
