Internal Medicine Physician

Coverage Needed:

• As soon as credentialed - September 30, 2025
• Schedule: Clinic hours from 8 AM to 5 PM

Clinical Details:

• Familiar with Medicare Advantage Experience- Care for Older Adults
• Clinic Setting Experience
• Transition of Care- Post Hospital Discharge
• Medicare Part C & D Star Measures Reporting, Medicare Annual Wellness, Acute Visits
• Patient Care: Providing medical care to patients, including conducting physical exams, diagnosing illnesses, developing treatment plans, and administering medications
• Patient Interaction: Communicating with patients and their families about their health conditions, treatment options, and preventative care
• Emergency Care: Providing emergency medical treatment when needed
• Age range of patient population: 65 and above (Medicare patients), some maybe under 65
• Average patients per day: 14-15
• Support Staff: Medical Assistant, Referral Coordinator, Clinic Admin , Front Desk, Lab
• EMR: EClinicals

Requirements:

• Florida license
• DEA License
• BLS Certified
• Must be local, there will be no travel reimbursement for this role
