Position: Licensed Practical Nurse

Location: Honolulu, HI

Pay: $37/hour

Experience: At least 1 year of Home Health experience.

Education: Licensed Practical Nurse; HI License.

Type: Full-time; Contract

Schedule: Day Shift, 8:00 AM – 4:30 PM (5 Days per week)

Kahu Malama Nurses is seeking a Licensed Practical Nurse (LPN) to join a growing and dynamic team!

Job Description:

• Provide direct patient care, including wound care, dressing changes, and tracheostomy management.
• Perform pressure ulcer assessments, health evaluations, and monitor vital signs.
• Administer oxygen therapy and manage medications, including refills.
• Educate and support patients and families in a home health setting.
• Coordinate care with RNs and physicians, reporting status changes.
• Document in MatrixCare and manage care planning.
• Conduct intake evaluations, interpret labs, and handle follow-ups.
• Assist with ambulation, crutch walking, and mobility support.

Position Requirements:

• BLS Certification (AHA).
• HI LPN License.
• Experience with wound vacs, JP/Hemovac drains, and ventilator support.
• Familiarity with MatrixCare and clinical documentation systems.
• Ability to monitor intake/output and manage case documentation.
• Knowledge of isolation precautions and infection control protocols.
