Job Title: Family / Internal / Geriatric Medicine Physician

Location: Olivia / Wabasso / Franklin, MN

Shift: Full-time position

Pay: $175 - $250 per hour

Summary:

We are looking for a Physician to serve patients in Long-Term Care (LTC) and Transitional Care Units (TCUs) across Olivia, Wabasso, and Franklin, MN. This position requires the provider to float between these locations to offer exceptional care to residents in post-acute and long-term care settings.

Responsibilities:

• Conduct comprehensive patient assessments and ongoing medical evaluations.
• Create and manage personalized treatment plans based on patient health needs.
• Diagnose and treat acute and chronic conditions, providing timely medical interventions.
• Prescribe medications, order diagnostic tests, and modify treatments as needed.
• Work closely with nurses, therapists, and social workers to coordinate comprehensive care.

Requirements:

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred.
• Valid medical license to practice in Minnesota.
• Previous experience in long-term care, transitional care, or geriatrics is a plus.
• Excellent problem-solving, communication, and teamwork skills.
