Position Summary

We are seeking a dedicated and compassionate Skilled Nursing Facility (SNF) Physician to provide high-quality medical care to residents across skilled nursing facilities in Northern Colorado.

This opportunity is ideal for a physician seeking a balanced work schedule, clinical autonomy, and the ability to provide meaningful patient-centered care with strong operational support. New graduates are welcome to apply.

Key Responsibilities

• Provide comprehensive medical care to residents in skilled nursing facilities.

• Evaluate, diagnose, and manage acute and chronic medical conditions.

• Develop and implement individualized treatment plans.

• Collaborate with nursing staff, specialists, and interdisciplinary care teams.

• Work 3-4 days per week with 10-hour shifts.

• Participate in 3-4 on-call shifts per month, including nights and weekends.

• Provide medical guidance and support during on-call coverage..

Qualifications

• Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.

• Board Certified or Board Eligible physician.

• Active medical license or eligibility to obtain licensure.

• Current BLS certification.

• Experience in skilled nursing, geriatrics, post-acute care, or internal/family medicine preferred.

• New graduates are welcome to apply.
