Job Details

What You Will Be Doing

Conduct patient assessments to develop personalized care plans
Deliver skilled nursing care in accordance with professional standards
Coordinate services with patients, families, and physicians
Maintain accurate and timely documentation for each case
Collaborate with interdisciplinary healthcare teams
Identify and address patient needs and care barriers
Support quality improvement and compliance initiatives

Skills You Need

Active and unrestricted NYS RN license
Strong written and verbal communication skills
Proficiency with Microsoft Office Suite
Valid driver’s license and access to a vehicle (not required for Manhattan-based roles)
