Job Title: Case Manager I

Job Location: Albuquerque /Rio Rancho, NM (Remote + Field)

Job Duration: 6 Months+ (Possible Extension)

Shift timings: Standard hours - 8:00 AM to 5:00 PM (Mon-Fri)

Job Description:

· This is a remote, temp-to-hire Case Manager position with required field visits, so candidates must reside in New Mexico, preferably in the Albuquerque/Rio Rancho area.

· The role includes travel to members' residences as needed, which may include homes, assisted living facilities, shelters, and other care settings.

· There may also be occasional support required in surrounding areas such as Santa Fe, Grants, Edgewood, Moriarty, and Valencia.

Position Summary:

· Client works with members, providers, and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long-term care, for members with high need potential.

· Client’s staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.
