Job Summary:

As a Licensed Practical Nurse (LPN) at [Facility Name], you will provide high-quality nursing care to residents in a long-term care setting. Key Responsibilities:

- Resident Care:

- Provide direct patient care, including administering medications and treatments as prescribed by physicians.

- Assist residents with daily living activities such as bathing, grooming, eating, and mobility.

- Monitor vital signs and report any changes in residents’ conditions to the nursing staff.

- Documentation:

- Maintain accurate and updated patient records, documenting care provided, observations, and any changes in resident conditions.

- Assist in the development and modification of individual care plans.

- Collaboration:

- Work closely with registered nurses, physicians, and other healthcare professionals to deliver comprehensive care.

- Communicate effectively with residents, families, and team members to ensure continuity of care and support.

- Health and Safety:

- Ensure a clean, safe, and comfortable environment for residents.

- Comply with all nursing standards, facility policies, and state regulations regarding patient care and safety.

- Education and Support:

- Provide education and support to residents and families regarding health management and wellness practices.

- Participate in care team meetings and training sessions as required.

- Emergency Response:

- Respond to medical emergencies and administer first aid or CPR as needed.

- Follow facility protocols for incident reporting and response.

Qualifications:

- Education:

- Completion of an accredited LPN program.

- Licensure:

- Current state licensure as a Licensed Practical Nurse.

- Experience:

- Experience in a long-term care setting is preferred but not required.

- Skills:

- Strong clinical assessment skills.

- Excellent communication and interpersonal skills.

Physical Requirements:
