Float LPN
Transitions LifeCare is seeking a full-time Float LPN to join our team! This position provides routine, PRN, and crisis care for hospice patients under the supervision of the assigned RNCM and/or IDG Team Leader. The Float LPN also offers phone support to patients and families, coordinates with the interdisciplinary team, and performs office duties. The role offers a Monday-Friday 8:00 AM – 5:00 PM schedule. Qualifying candidates with at least 2 years of clinical experience may be eligible for up to a $5,000 sign-on bonus.

Responsibilities
- Provide and document safe, competent, compassionate hospice nursing care for assigned patients under RN or physician supervision.
- Offer telephone support to patients, families, physicians, and partners, assisting with immediate needs within the LPN scope of practice.
- Review supplemental orders, perform chart audits, and assist with office duties as assigned.
- Provide end-of-life education and information to patients and families in compliance with CMS Conditions of Participation.
- Collaborate with the IDG to assess patient needs and coordinate care.
- Participate in performance improvement activities and orient/train new employees.

Requirements
- Licensed Practical Nurse with current NC license.
- Minimum 2 years of recent clinical experience (Hospice, home health, or nursing home experience preferred).
- Hospice Nurse Certification preferred.
- Knowledge of Medicare regulations, licensing laws, and accreditation standards.
- Proficient in computer skills and CPR certified.
- Reliable transportation required.
- Must be able to travel and provide care in patient homes or facilities within the service area.
Physical Demands
- Ability to lift and/or carry up to 50 pounds.
- Ability to perform physical assessments and communicate effectively with patients, families, and physicians.
