A hospital in Rockland County is currently seeking a licensed Nurse Practitioner to join their team for an exciting opportunity at their Pride Center. About the Opportunity: Schedule: Monday to Friday- Open to full or part time Specialty: Primary Care- Pride Center Responsibilities: The Nurse Practitioner (NP) will: Provide direct care and consultations to patients Serve as a consistent Care Coordinator through the continuum of care Assess the physical, psychological, spiritual, and social needs of the patient Direct patients to the next appropriate service and track their progress Assure that all patients receive appropriate evaluation, diagnosis, treatment, medical screening, and medical evaluation whenever indicated Perform other duties, as needed Qualifications: NY Nurse Practitioner (NP) license Desired Skills: Experience treating the LGBTQ community
