Location: Palo Alto & Surrounding Areas, CA

Job Type: Per Diem (Evenings, Nights, Weekends)
Compensation: $45–$48 per visit (DOE) + $5/hour standby + mileage reimbursement

Position SummaryWe are seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for per diem after-hours coverage including evenings, nights, and weekends. In this role, you will provide skilled nursing care to hospice patients, support their families, and collaborate with an interdisciplinary team to ensure quality and comfort during end-of-life care.
Key ResponsibilitiesDeliver direct patient care following the hospice plan of care and physician orders
Conduct in-person visits during after-hours or weekends to manage symptoms and provide urgent interventions
Complete EOBs, consents, and submit documentation to intake promptly
Accurately document visits in the EMR within 24 hours
Supervise nursing assistants (as allowed by state regulations)
Administer medications and treatments and monitor patient responses
Escalate concerns and changes in patient condition to RN Case Managers or Clinical Services Manager
Stay current on hospice best practices through ongoing training
Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) – Required
1–2 years of clinical nursing experience (hospice preferred)
Current driver's license, insured personal vehicle, and ability to travel within assigned area
Fire Safety Certification – Required or willingness to obtain
Strong clinical judgment, organizational skills, and compassionate bedside manner
Per Visit Rate: $45–$48 (based on experience)
$5/hour standby pay
