Family Practice/Primary Care Nurse Practitioner

A hospital in Rockland County is currently seeking a licensed Nurse Practitioner to join their team for an exciting opportunity at their Pride Center. About the Opportunity:

• Schedule: Monday to Friday- Open to full or part time
• Specialty: Primary Care- Pride Center

Responsibilities: The Nurse Practitioner (NP) will:

• Provide direct care and consultations to patients
• Serve as a consistent Care Coordinator through the continuum of care
• Assess the physical, psychological, spiritual, and social needs of the patient
• Direct patients to the next appropriate service and track their progress
• Assure that all patients receive appropriate evaluation, diagnosis, treatment, medical screening, and medical evaluation whenever indicated
• Perform other duties, as needed

Qualifications:

• NY Nurse Practitioner (NP) license
