Job Title: Licensed Practical Nurse (LPN) – Temporary Medical Professional Services

Location: Individual’s Home, Slater, MO

Agency: Missouri Department of Mental Health – Northwest Community Services (NWCS)

Contract Start Date: August 1, 2025

Duration: Approximately 6 months (with option to extend)

Shifts Available:

Day Shift: 6:00 AM – 6:00 PM (2 positions)

Night Shift: 6:00 PM – 6:00 AM (2 positions)

Work Schedule: 3 twelve-hour shifts one week; 4 twelve-hour shifts the following week Includes overnights, holidays, and weekends

Job Summary:

NWCS is seeking four (4) Licensed Practical Nurses (LPNs) to provide direct nursing care in a residential setting. The role is critical in supporting individuals with daily living activities and complex medical needs in a compassionate, skilled, and professional manner.

Key Responsibilities:

• Assist individuals with Activities of Daily Living (ADLs): feeding, bathing, dressing, shaving, bed-making, etc.
• Check and record vital signs, height, weight, fluid intake and output
• Provide detailed condition reports for each individual
• Deliver specialized medical care including:
• Trach care (change inner cannula, strap replacement, suctioning, humidification/HME use)
• G-tube and suprapubic catheter management
• Nebulizer treatments , blood glucose monitoring, and insulin administration as needed
• Administer medications as required
• Work collaboratively with NWCS staff on-site
