A 28-Day Rotation Designed for Flexibility & Fulfillment

• Bi-weekly schedule: 

  • 7 days on Monday- Sunday 4:30pm-8:30am, 11 days off 
  • 3 days on Friday at 4:30pm through Monday at 8:30am
    
    
• Extended time off = More time for family, hobbies, rest, and self-care
• Designed to optimize both performance at work and quality of life at home

 About the Role: Extended Care RN

In this rewarding after-hours, on-call role, you will:

• Triage and manage after-hours patient needs via phone and onsite visits
• Conduct hospice informational visits and nursing admission assessments
• Support patients and families by collaborating on individualized Plans of Care
• Work alongside an interdisciplinary team to educate, advocate, and provide holistic, compassionate care
• Deliver exceptional service while making a real impact during the end-of-life journey
