Description
\n Provide skilled nursing, care, and case management for assigned patients in their place of residence or at our in-patient Hospice Center. Admits patients onto services and coordinates to ensure continuity of care. Oversees care provided by aides and evaluates compliance with care plans. Maintains accurate patient records and participates in quality improvement activities.
