Location: Mishawaka, Indiana (near South Bend & 80 miles from Chicago)
Setting: Program of All-Inclusive Care for the Elderly (PACE)
Focus: Frail & Elderly Population Community-Based Senior Care
A growing, established PACE program in Northern Indiana seeks a Board-Certified Family or Internal Medicine physician with geriatric experience to serve as Medical Director. This leadership role offers the chance to guide an interdisciplinary team and make a meaningful difference in the lives of vulnerable seniors.
Role HighlightsProvide clinical leadership across clinic, home, and community-based care
Supervise and mentor Primary Care Providers and APPs
Lead Quality Assurance & Performance Improvement (QAPI) initiatives
Develop medical policies, protocols, and care standards
Participate in care plan meetings, intake evaluations, and family conferences
Perform new and follow-up physical exams and assessments
Represent the program at community and partner meetings
Shared provider on-call responsibilities
Salary: $231,000 $280,000 (based on experience)

Valid driver's license, vehicle, and insurance
