pDescription/ppstrongPOSITION: 2nd Shift Caregiver/strong/ppstrongREPORTS TO: /strongExecutive Director/House Manager/ppstrongJOB SUMMARY: /strongAssist in the daily living activities, monitoring, and supervision of the residents. Perform housekeeping, life enrichment, and dining tasks as required. This position is responsible for representing Frontida well and promoting its culture./ppstrongDUTIES:/strong/ppThe Caregiver will be responsible for the below job duties including but not limited to:nbsp;/pulliRepresent Frontida with professionalism, always embracing Frontida’s culture, which includes “treating others as you would like to be treated.”/liliAssist residents in activities of daily living (i.e., toileting, bathing, eating, etc. per individualized service plan/liliAssist in maintenance of physical Community daily (i.e., cleaning, straightening, laundry, dishes etc.)./liliMust be able to assist semi-ambulatory residents with transfers and personal care, which may, at times, require lifting and gait-belt use./liliCook meals and provide snacks for the residents, following the menu plan./liliCommunicate facility needs list to the Executive Director/House Manager./liliPut groceries away, ensuring all items are properly dated and stored./liliMonitor and chart administration of residents’ medications as applicable./liliDocument accidents and incidents using incident reporting form. in electronic record and report all incidents or observed concerns (conflict, potential misconduct etc.) to Executive Director /Designee./liliDocument resident daily observations (i.e., daily activities, behaviors, changes, etc.)./liliFacilitate daily activities, in accordance with life enrichment schedule./liliProvide comfort and companionship to the residents, treating all residents with dignity, warmth and respect. with reference to Resident’s Rights/liliCommunicate concerns to the Executive Director/designee immediately./liliCall 911 if the resident is in distress./liliFollow emergency procedures if an emergency occurs./liliWork scheduled shifts as assigned; if unable, contact supervisor immediately./liliBe available to attend mandatory monthly staff meetings and training as requested by the Executive Director/House Manager and to achieve regulatory required education compliance./liliBe available for occasional hours at another Frontida community location./liliResponsible for reading any new or updated ISP’s on an as-needed basis./liliResponsible for clearing all walkways of snow, ice, or any other debris./liliProvide 24-hour awake care for residents./liliFollow all guidelines according to DHS 83./liliAll other duties as assigned by the Executive Director/House Manager./li/ulpIn addition to my duties as outlined above I understand that I am expected to uphold Frontida’s values in my day-to-day activities./pulliTell the Truth/liliWork Hard/liliFollow the Rules/liliLove Others/liliHave Fun/li/ulpRequirements/ppQUALIFICATIONS: Must complete the probationary period successfully. Must be able to be flexible and work alone if necessary. Desired: High School Diploma or equivalent; CBRF Training completed. Must actively demonstrate Frontida’s culture and exhibit extraordinary care toward Frontida’s residents./ppPHYSICAL REQUIREMENTS:/pulliStand, sit and walk for long periods of time./liliFull ranges of motion to climb, balance, kneel, crouch or crawl./liliFull function of hands and arms/liliOccasionally lift/or move up to 25 pounds with occasional lifting of residents at higher weight./liliAble to transport residents in wheelchairs.nbsp;nbsp;/li/ul
