Job Description
Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, outpatient care and assisted living memory care. As we continue to expand our services & locations, our community has grown far beyond Paris. Willowbrook is Horizon Health’s newest 14 bed Memory Care Facility that seeks to provide our residents with an experience that makes them feel like they are living right at home. We have the ability to provide more one - one family-oriented care with our specialty trained and dedicated staff.
Position Summary:
Provide quality care to residents; implement specific procedures and programs; report pertinent information to the Resident Director; respond to inquiries or requests for information; assist teammates and/or Resident Director with tasks to support operations.
Essential Functions (Responsibilities/Accountabilities):
1. Provide quality nursing care to residents in an environment which promotes their rights, dignity, freedom of choice, and their individuality as illustrated by the following:
2. Provide individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
3. Provide proper medication administration and clinical assessment to each resident as outlined in their service plan, and as ordered by their physician.
4. Knowledgeable of the individualized service plan for residents and provide support to the resident according to the service plan. Contribute to the service planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents' needs and preferences.
5. Assist with individual needs of residents which may include assistance with grooming, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements.
6. Provide housekeeping, laundry, activities, and food preparation and dining services to the residents and facility.
7. Maintain the comfort, privacy, and dignity of each resident in the delivery of services to their environment.
8. Fully understand all aspects of residents' rights, including the right to be free of administrative staff incidents or evidence of resident abuse or violation of residents' rights.
9. Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the Residence Manager such as lighting or equipment problems.
10. Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, missing items according to established policy and participate in efforts to locate missing items.
11. Observe residents for changes in condition or behavior and promptly report these changes to appropriate Physician, family and Residence Director.
12. Provide care that maintains each resident's skin integrity to prevent pressure ulcers, immobile residents, and applying moisturizers to fragile skin, etc.
13. Answer residents' call bells promptly and courteously.
14. Perform various tasks assigned by the Resident Director including vital signs, resident's weights, applying creams/ointments, collecting specimens, etc.
15. Assist with orienting residents and their families to the nursing home upon admission and to the unit when transfers occur.
16. Lift, move, and transport residents, using proper body mechanics or for accident prevention.
17. Communicate and interact effectively and tactfully with the residents, visitors, families, peers, and supervisors.
18. Assist and escort residents for appointments such as beauty shop, activities, church services, etc. Participate in activities and functions as directed.
19. Practice careful, efficient, and no-wasteful use of supplies and linen and follow established charge procedures for resident charge items.
20. Perform all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
21. Walk extensively to and from various locations of the unit while making rounds and transport residents within the facility.
22. Support an atmosphere, which promotes a homelike environment.
23. Respond to inquiries or to requests from residents, visitors, other personnel, etc. within given time frames and within established policy.
The responsibilities listed above are not all-inclusive; other activities may be required in support of the hospital’s goals and objectives.
Position Requirements:
- Education/Training: Must be a graduate of an accredited school of Nursing and hold a
- valid, current license in the State of Illinois.
- Skill(s): Moderate reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; auditory and visual skills; ability to bend, stoop, sit, stand, reach, and lift items weighing 50 pounds or less; satisfactory completion of post-offer, preemployment physical as required.
- Experience: A minimum of three months' prior nursing experience in assisted living preferred.
Pay Range:
Pay ranges from $23.855/hour to $38.168/hour for an RN (rate of pay is based on applicable years of experience)
Pay ranges from $18.122/hour to $28.329/hour for an LPN (rate of pay is based on applicable years of experience)
