JOB TITLE: Case Manager - Quakertown & Langhorne Offices (position will travel between both offices weekly)

DEPARTMENT: Family Strengthening Programs

REPORTS TO: Family Strengthening Manager

SUMMARY: The Case Manager engages the client in a family-centered assessment and goals-oriented service that supports and empowers the parent or caregiver in the following areas: parenting skills, knowledge, and abilities. Services are delivered utilizing evidence-based or evidence-informed curricula without neglecting the other social determinants of health and other stressors that may lead to risk of child abuse and neglect.

DUTIES AND RESPONSIBILITIES:

  • Complete training in evidence-based/evidence-informed programs such as Nurturing Parenting and Parents As Teachers.
  • Utilize the training to deliver curriculum-based services to clients in their homes or communities on a weekly basis, to meet unit expectations of the department. Services may need to occur outside of traditional work hours based on client needs.
  • Co-facilitate groups on a weekly/monthly basis as it applies to meeting program requirements.
  • Provide warm handoffs for resources to address additional concerns, including but not limited to: mental health/substance use treatment, employment or educational challenges, linkages to early intervention services, housing or utility related services.
  • Work collaboratively and communicate clearly with the local child welfare agency to address risk factors identified and provide updates that are factual on the progress of the parent or caregiver’s parenting skills, knowledge, and abilities, as appropriate and necessary.
  • Communicate effectively with referral sources in order to ensure continuity of care and development of relationships with referral sources.
  • Complete documentation in all required data systems in a timely manner and in accordance with agency expectations, within two business days.
  • Attend quarterly conference meetings at the Bucks County Children & Youth office to review cases assigned that are also engaged with a Children & Youth case manager.
  • Attend court hearings to relate a parent/caregiver’s status with program compliance and any risk factors that were reduced and those that may remain a risk to the child.
  • Participate in individual and group supervision.
  • Understand client’s rights to privacy and confidentiality as articulated by agency policy and procedures.
  • Performs other related duties as assigned by management.

SUPERVISORY RESPONSIBILITIES:

  • This job has no supervisory responsibilities.

QUALIFICATIONS:

  • Bachelor's Degree (BA) from four-year college or university, in health or human service or closely related field.
  • Experience working with parents/families engaged in child-welfare services, or case management experience a plus.
  • Valid Driver’s License (issued by the state in which the employee lives) and a safe driving record with proof of insurance coverage.
  • Computer skills required: Microsoft 365 (Outlook, Word), Credible EHR, Teams, Zoom, basic data entry in PA Home Visiting Database.
